Sports Physicals Information
PHYSICALS will once again be offered at a reduced rate by the Freeport Health Network. Physicals are required for all junior high and high school students participating in sports. The sports physicals will be given on Tuesday, May 20, from 12:30 p.m. to 4:00 p.m. Physicals will be given at the Forreston Jr/Sr
High School. Call 815-938-2175 for appointments. The cost is $25 and you must pay for your physical that day. If you are paying by check, please make
the check payable to FHN. IHSA/IESA physical forms, and consent forms for physicals are available in the high school office or FVV website. These
sports physicals are only good for sports and covers 396 days. If you take advantage of this, your sports physical will be good from 5/20/2025 to 6/20/2026.
This will cover the entire school sporting seasons. This sports physical will not qualify for 6th and 9th grade entry for school. (School entry physicals for
6th and 9th grade will work for sports physicals.) Sports physicals are not needed to attend camps.
FHN donates all proceeds from the sports physicals back to the sports department.
